Rahbar Medical & Dental College (RMDC) located at Link Zarar Shaheed / Harbanspura Road, near Jorray Pull Lahore Cantt.
Requirements for Submitting forms:-
- 10 x passport size recent photograph including 4 x 1”x 1” size with blue background
- 4 x copies of CNIC of student or B Form
- 4 x copies of CNIC (Father/Guardian)
- 4 x copies of Domicile along with Original
- 4 x copies of Matric, F.Sc, A-Level, O-Level and equivalency certificate along with all Original certificates (where applicable)
- Migration certificate in original if student migrated from another board
(out of Punjab) - Undertaking on Judicial stamp paper of Rs 100/- duly attested by Oath Commissioner
- Medical certificate from medical officer holding valid PMC registration
- As per PMC directive students are required to deposit attested copies of wealth statement of their parents/guardians (equivalent to five year tuition fee), income tax return and life insurance policy of parents/guardians (covering the expenses of education of student for 5 years) in case of death of parents/guardians
- For facilitation of parents, a counter/desk of registered insurance company will be established in admission cell RMDC.
Note: (Please submit 1 set of Original documents with Undertaking and 2 sets of photocopies of all documents separately in plastic file pouch. All documents shall be attested by grade 17 and above Government officer/Army officer).
